SUMMARY:
Works under the general supervision of the Clinical Team Leader of Hospice. Follows the established plan of care. Completes home visits to provide palliative care visits, treatments and teaching. Reports changes in status to the Clinical Team Leader.
ESSENTIAL JOB FUNCTIONS:
Nursing
- Provides nursing care to patients in homes.
- Responsible for case management of patients.
- Responsible for the implementation of services to meet client needs. Follows current nursing professional standards of practice and agency approved protocols for hospice.
- Provides ongoing assessment of clients, assuring an accurate evaluation of client needs. Identifies when clients need or quality for additional services and makes referral to appropriate SI manager or other community resource. Assesses client and caregiver needs, provides client and caregiver education.
- Maintains current and accurate client records through use of laptop computers and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines.
- Supervises and monitors the performance and skills of Home Health Aides, and LPN’s.
- Collaborates with other disciplines and performs nursing activities within the plan of care.
- Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third-party payers.
- Completes home visits and on-call duties after normal business hours, on weekends, and holidays, and other positions temporarily, as needed or assigned.
- Performs other related duties that may become necessary.

Employee Responsibilities:
Follows policy and procedures on TB testing.
Follows policy and procedures on annual influenza vaccinations.
Attends all mandatory in-services
Minimum Requirements: Registered Nurse, currently holding an unencumbered license to practice in the state of Montana; BSN preferred. Current CPR certification (Basic Life Support).
Knowledge, skills, and abilities required:
- Ability to understand verbal instructions.
- One year of nursing experience, preferably in Hospice, Home Health, or a Skilled Nursing Facility
- Basic computer literacy with experience in Windows-based programs preferred.
- Must possess strong oral and verbal communication skills, leadership ability, emotional stability, flexibility, and high professional standards of care.
- Must possess a valid state driver’s license.
PHYSICAL DEMANDS:
Sits, stands, bends, lifts, and moves intermittently during working hours.
Some driving required.
Must be able to lift, push, pull and move medical supplies, etc. 25-50 lbs.
WORKING CONDITIONS:
- Work involves some exposure to hazards, blood borne pathogens, or physical risks, which require following basic safety precautions.
- Normal health care environment.
- Work involves exposure to plants and animals within the environment.
- Works in clients’ homes and in an office area.
