Nurse Supervisor Full-Time 1st Shift
The Memory Care Nurse Supervisor is responsible for ensuring the well-being of Memory Care residents through the delivery of high-quality, person-centered nursing services. This position ensures compliance with all applicable state and federal regulations and works in collaboration with the Assisted Living Nurse Manager to support a safe and caring environment for residents.
Current Licensed Practical Nurse (LPN) license in the Commonwealth of Virginia.
UAI, ISP, and CDP certification (or willingness to obtain within 6 months).
Minimum 2 years of experience in gerontological or senior care nursing.
Strong organizational and time management skills.

Supervise, coach, and counsel Memory Care team members.
Assist the Nurse Manager with delivery of nursing services for Assisted Living (AL) and Memory Care (MC) residents.
Coordinate physician services within the Memory Care neighborhood.
Review the 24-hour report daily and follow up on unresolved issues.
Oversee medication management, including cart reviews, refills, and proper disposal.
Conduct weekly skin assessments and monitor ongoing conditions with RMAs and CNAs.
Develop, update, and maintain:
Individualized Service Plans (ISPs)
Uniform Assessment Instruments (UAIs)
Fall and Risk Assessments
Promote strong communication among staff, residents, and families.
Respond to resident and family concerns in a timely manner.
Ensure proper use and understanding of care plans by nursing staff.
Maintain confidentiality of resident information at all times.
Monitor care delivery systems and address trends with corrective action plans.
Maintain compliance with federal/state regulations and internal policies.
Assist with audits, inspections, and incident investigations.
Participate in monthly care plan meetings with families and the interdisciplinary team.
Partner with Program Manager to support resident engagement and recreational activities.
Build and maintain professional relationships with physicians, pharmacies, dietitians, and therapy providers.
Assist in developing staffing and scheduling plans that ensure consistent care.
Provide medication coverage for breaks and callouts as needed.
Participate in on-call rotation for higher levels of care.
Help coordinate admissions and level-of-care transitions.
Assist in policy development or revision as requested by the Administrator.
