POSITION SUMMARY
The Clinical Pharmacist II provides advanced pharmacy services, including drug information, pharmacokinetics, drug therapy monitoring, patient education, medication reconciliation, transition of care evaluations, and ensures compliance with controlled substance regulations. Responsible for directly supervising pharmacy support personnel and actively engaging and collaborating with all hospital personnel. Actively fosters an environment of service and care by striving to provide exceptional satisfaction for patients, families, physicians, nurses, and staff. REQUIRED QUALIFICATIONS
Education: PharmD degree or BSPharm with relevant experience post- 2006.
Skills: Experience in hospital pharmacy services, including unit dose, IV admixtures, medication reconciliation, and addressing provider questions.
Years of Experience: Minimum 1 year (full-time equivalent of 2,040 hours) in inpatient pharmacy practice. License: License: Must be a registered pharmacist, licensed in good standing in Michigan and Ohio. Single facility based pharmacists must retain state licensure per site location.
Certification: BLS (Basic Life Support)
PREFERRED QUALIFICATIONS
Education: Completion of a PGY1 Pharmacy Residency Training Program.
Skills: Expertise in patient counseling, pharmacokinetic evaluations, preceptorship, antimicrobial stewardship basics, evidence-based literature review, and time management. Experience in leadership roles or multidisciplinary team collaboration is highly desirable.
Years of Experience: Minimum 3 years (full-time equivalent) in inpatient pharmacy practice.
License: no additional preferred
Certification: ACLS (Advanced Cardiac Life Support), Board Certification, or two professional training certificate programs for acute care.
WORKING CONDITIONS
Personal Protective Equipment: Required when working with patients or hazardous medications.
Physical Demands: Must be able to lift light materials. Non-material handling of continuous sitting and reaching; frequent standing; occasional squatting, kneeling, walking, and stairs. Must be able to move about and between workstations.
